The 19th of May of every year is “World IBD Day”, which is dedicated to raising awareness of Inflammatory Bowel Disease (IBD) worldwide. People are often hesitant to talk about their bowel habits and seek medical care to correct the changes in their bowel habits, including persistent diarrhoea, passing blood in stools, grumbling sounds from the abdomen, and abdominal pain. They end up suffering in silence.

Before two decades, most doctors considered IBD a Western disease. However, India is witnessing a steady increase in IBD cases in children and adults. World IBD Day provides a chance to raise awareness about this disease.

What is IBD?

IBD is a chronic gastrointestinal disease characterised by inflammation (redness and swelling) and ulcers in the affected parts of the digestive system. Crohn’s disease and ulcerative colitis are combinedly called IBD. The main difference between these two conditions is that Crohn’s disease can affect any part of the gastrointestinal tract (mouth to anus), while ulcerative colitis is limited to the large intestine (colon and rectum).

Are IBD and IBS the same?

IBD should not be confused with IBS or Irritable Bowel Syndrome, which is another condition that affects the intestines.

Why is it important to diagnose IBD early?

A delay in diagnosing IBD can lead to complications due to permanent damage to the intestines. Moreover, IBD can also increase the risk of developing colorectal cancer.

What are the symptoms of IBD?

The symptoms of IBD can be confused with the symptoms of recurrent digestive infections, abdominal tuberculosis, or recurrent diarrhoea. Symptoms to look out for are:

What causes IBD?

The exact cause is unclear. But people with IBD may have immune systems working differently or incorrectly against the environmental triggers, including bacteria and viruses.

What are the risk factors for IBD?

How to diagnose and treat IBD?

It is important to seek medical care from a medical gastroenterologist when there are changes in bowel habits, as these symptoms are often confused with other digestive disorders. A thorough evaluation of symptoms and relevant tests can help diagnose IBD and provide the right treatment.

Early diagnosis and timely treatment can help the vast majority of patients with IBD lead a normal life. Thus, raising awareness of IBD can help provide early diagnosis and timely intervention, which are key to altering disease outcomes for the better.

Takeaway points

IBD is a chronic gastrointestinal condition mainly characterised by changes in bowel movements. Untreated IBD can lead to other complications and increase the risk of colorectal cancer. Therefore, it is important to raise awareness about IBD for early diagnosis and timely treatment.

About 20% to 30% of pregnant women experience diarrhoea at some point during pregnancy. The World Health Organization defines diarrhoea as passing 3 or more loose or liquid stools in a day or more frequent bowel movements than usual for a person with changes in stool consistency.

What are the types of diarrhoea seen during pregnancy?

What causes diarrhoea during pregnancy?

Reasons unrelated to pregnancy:
Acute diarrhoea
Chronic diarrhoea
Reasons related to pregnancy:

What are the complications of diarrhoea during pregnancy?

Dehydration is the most common complication seen in pregnant women with diarrhoea. Excessive thirst, dry mouth, passing dark yellow-coloured urine, and reduced urine output are the symptoms of dehydration.

Severe diarrhoea lasting for more than 2 days or diarrhoea accompanied by the passage of blood and mucous needs urgent medical attention.

How to manage diarrhoea during pregnancy?

When to consult a doctor?

Consult a doctor immediately if the diarrhoea is severe, lasting more than 2 days, or if you are excessively dehydrated and passing blood or mucous in stools.

How is it treated?

Doctors will prescribe antibiotics if you are suffering from bacterial gastroenteritis. They may also advise probiotics. Extremely dehydrated patients might need admission and intravenous infusion to replenish blood volume.

Is your stomach rumbling frequently? Is your rumbling stomach making you worked up? Are those growling noises from the stomach causing social embarrassment? Do you want to know if these noises are normal? This blog will answer all these questions and educate you about these noises and how to avoid them.

What is Borborygmi?

The noises coming from the stomach or abdomen are called borborygmi in medical terminology. These noises are produced when foods, water, and gases move through the stomach, small intestine, and large intestine because of continuous muscular movements in the digestive system, called peristalsis.

Are these noises normal?

These noises produced by pushing digestive contents forward are entirely normal and can be seen in everyone.

When do these noises become a problem?

These noises can become problematic if you notice them more frequently, become increasingly loud, and interfere with your work and social life.

What are the common causes of abnormal stomach noises?

How to reduce excessive noises coming from the stomach?

When should you consult a specialist?

Consult a medical gastroenterologist if the excessive noises coming from the stomach are associated with the following:

Takeaway points

The noise from the stomach or abdomen is due to the movement of foods, water, and gases through the digestive system because of continuous muscular movements. They are entirely normal.

Why is acid reflux common during pregnancy?

About 80% of pregnant women experience gastroesophageal reflux disease (GERD) during pregnancy. GERD, also commonly referred to as heartburn or acidity, involves regurgitation or backflow of stomach contents into the food pipe or oesophagus.

Acid reflux occurs due to improper functioning or frequent opening of the gastroesophageal sphincter or valve at the base of the oesophagus that acts as a gate between the food pipe and stomach. Weakness in the diaphragm, conditions like hiatus hernia, pregnancy, unhealthy food habits and lifestyle are other reasons for acid reflux.

What causes GERD during pregnancy?

Hormonal changes in women during pregnancy are the most common causes of acid reflux. Increased levels of progesterone during pregnancy cause the ineffective closure of the sphincter, which prevents the entry of stomach contents into the food pipe. As a result, the food and gastric acid keep coming back into the food pipe, causing symptoms.

Additionally, the growing foetus and enlarging uterus in the abdomen put pressure on the stomach and its contents resulting in reflux.

What are the symptoms?

Heartburn is the most common symptom experienced by pregnant women suffering from GERD. Other symptoms include:

What are the tips for managing GERD during pregnancy?

When to seek medical help?

Consult a medical gastroenterologist if you are experiencing reflux symptoms despite changes in diet and lifestyle.

What are the treatment options for managing GERD during pregnancy?

Antacids are usually prescribed for pregnant women to neutralise stomach acid. Drugs to reduce stomach acid production are also prescribed in some cases.

Tuberculosis (TB) is a devastating infectious disease caused by a bacteria called Mycobacterium tuberculosis, which kills over 4,000 people every day globally. Unlike other infections, TB bacteria affect our body slowly over a prolonged period, causing symptoms for months to years.

How can you get tuberculosis?

TB bacteria spread through droplets from the cough or sneeze of one person to another. However, not all people exposed to these droplets develop TB. The bacteria affects only vulnerable people with weakened immune systems, chronic liver disease, HIV, uncontrolled diabetes, nutritional issues and slightly older people.

On the other hand, some people might have TB in their bodies in an inactive form. This kind of inactive TB becomes active or reactive when the person’s immune system weakens.

What is abdominal tuberculosis?

Most people know TB as an infection that affects the lungs. But not many know that TB bacteria can also affect other body parts, including the digestive system. Many gastroenterologists come across cases of TB involving the digestive system, called abdominal TB. It can affect the food pipe or oesophagus, stomach, small intestine, and large intestine. It is unique because it is not easy to detect and produces non-specific symptoms depending on the part it affects.

TB affecting the intestines is more common than TB affecting the food pipe or stomach. It mainly affects the part of the intestines called the ileocaecal junction, where the small intestine meets with the large intestine. The concentration of the lymphoid tissue and the mixing of the contents of small and large intestines in this area make it favourable for the growth and multiplication of TB bacteria.

Is abdominal tuberculosis contagious?

Abdominal TB does not pass from one person to another unless the patient also has a TB infection in the lungs.

What are the symptoms of abdominal tuberculosis?

What are the non-specific symptoms of abdominal tuberculosis?

The following symptoms can make it challenging to diagnose abdominal TB, as some other digestive conditions can also produce similar symptoms.

Why is abdominal tuberculosis called a "great mimicker"?

Abdominal TB can mimic other conditions like peptic ulcers, colorectal cancer, inflammatory bowel disease called Crohn’s disease, certain chronic infections or inflammatory conditions. These reasons make abdominal TB a great mimicker.

What are the complications of abdominal tuberculosis?

Abdominal TB should be promptly diagnosed and treated, as it could cause the following complications.

How to diagnose abdominal tuberculosis?

The diagnosis of abdominal TB often gets delayed as its symptoms are confused with other gastrointestinal issues. People with abdominal TB symptoms should seek help from a medical gastroenterologist, who will perform a series of tests to diagnose it.

How to treat abdominal tuberculosis?

Takeaway points

How can Bangalore Gastro Centre help you?

Bangalore Gastro Centre Hospitals in HSR Layout houses top gastroenterologists highly experienced in rightly diagnosing and managing abdominal TB. The best gastro doctors at this facility also offer endoscopy and colonoscopy tests to track the disease activity and treat the infection accordingly.

Colonoscopy involves examining the insides of the large intestine using a device called a colonoscope, which is sent inside the body through the anus. A colonoscope is a flexible and long tube fitted with a camera at one end to help the examiner visualise the insides of the intestine and look for any swollen or irritated tissues, ulcers, polyps, or cancer. The colonoscope can also excise pre-cancerous or cancerous polyps from the lining of the intestine to treat cancer.

Why is disinfecting colonoscopes after each use important?

Colonoscopies can screen people at risk of colon cancer and determine the reasons for digestive issues like chronic diarrhoea or constipation and rectal or anal bleeding. The procedure can be life-saving. However, they can also harm people by causing life-threatening infections, especially when the colonoscopes are not properly disinfected or decontaminated in between uses.

The uncleaned and inadequately disinfected colonoscopes can transmit disease-causing microorganisms from one person to another. Thus, colonoscopes should be disinfected and decontaminated between each use. Studies have shown that around 15% of colonoscopes at some colonoscopy centres harbour bio-dirt with harmful microorganisms. The sloppiness of colonoscopy centres and lack of quality control measures could be the reasons for inadequate decontamination.

How is a colonoscope decontaminated and disinfected?

Colonoscope cleaning after each use is the top priority for experts at Bangalore Gastro Centre Hospitals. It is one of India’s few colonoscopy centres with a dedicated decontamination unit to perform high-standard endoscope cleaning and disinfection procedure. The centre uses a specialised and fully automatic endoscope reprocessor, Olympus OER-AW, which is of international standards. This reprocessor allows for easier and quicker reprocessing and eliminates more than half of the manual cleaning steps.

Untitled design (19)

The decontamination procedure starts with manual cleaning of the scope immediately after the colonoscopy procedure. This step involves precleaning, rinsing and flushing the scope using water. It is followed by brushing all accessible channels to remove debris and contaminants. It is further decontaminated in an automatic endoscopic reprocessor to achieve 100% decontamination.

The automated decontamination process starts with placing the colonoscope in the reprocessor, connecting it to all required tubes and ports, and setting a program to clean. The disinfectant used in this machine is an activated glutaraldehyde solution called CIDEX, which is 2.45% glutaraldehyde. The shelf life of this disinfectant is 14 days, and the efficacy of this solution is monitored using CIDEX solution strips before placing the scope in the machine.

Untitled design (18)

The scope is soaked in this solution for 20 minutes for high-level disinfection. The external and internal channels of the scope are then exposed to multiple enzymes in every reprocessing cycle. The machine takes 34 minutes and 57 litres of RO water for each cycle of high-level disinfection and decontamination. This process takes place in a separate decontamination unit at Bangalore Gastro Centre. Most centres without automated endoscopic reprocessors wipe the colonoscopes with gauze, immerse the scope in CIDEX solution for a couple of minutes, and use the same scope for other patients. This kind of disinfection increases the risk of infections in patients. The automated decontamination process distinguishes the Bangalore Gastro Centre Hospitals from other colonoscopy centres.

The patients using colonoscopy services at Bangalore Gastro Centre also feel very comfortable as they are greeted with a smile, followed by the introduction of the endoscopic technician and staff nurse. Most patients feel anxious about the procedure. The staff makes them feel comfortable by creating a homely environment in the centre and addressing their queries. The team also keeps them informed of each step by explaining them properly to make them feel relaxed.

Visit Bangalore Gastro Centre Hospitals in HSR Layout if you are looking for a colonoscopy test in Bangalore for constipation treatment or colon cancer treatment, or treatment for any other digestive issue. It is the best gastro hospital which houses top gastroenterologists and endoscopic technicians. According to google patient reviews, it is also the top-rated gastro hospital in Bangalore. Gastroesophageal reflux disease (GERD) is a digestive condition which involves the regurgitation of stomach contents into the throat and mouth through the food pipe. On the other hand, asthma is a respiratory disease characterised by difficulty breathing due to the narrowing of airways. However, both GERD and asthma trigger cough in patients. Sometimes, GERD can produce asthma-like symptoms in people who never had asthma.

Research shows that about 25% to 80% of people with asthma also have GERD. The relationship between GERD and asthma is believed to be two-way, where GERD can trigger asthma flare-ups and vice-versa. Although the exact relationship between these conditions is not clear, scientists believe it could be because of the following reasons:

How to know if your asthma symptoms are due to GERD?

Some people with GERD do not suffer from heartburn, the hallmark sign of acid reflux. Instead, they may experience a dry cough, especially at night, and difficulty swallowing, which signals asthma. The following signs can help determine if your asthma is due to GERD:

How to break the cycle of GERD worsening asthma and asthma worsening GERD?

Treating GERD or acid reflux symptoms may help people suffering from GERD and asthma. A doctor may recommend proton pump inhibitors like omeprazole or esomeprazole for a month or two to control stomach acid secretion. Controlling GERD symptoms in such people may also reduce asthma symptoms and the need for asthma medications. People not benefitting from GERD medications may be referred to a gastroenterologist, for further treatment, including surgery.

Following some dietary restrictions and lifestyle changes, including avoiding foods and drinks triggering acid reflux, eating small meals, avoiding alcohol and smoking, losing weight, using pillows to raise the head, and loose clothing can also help control GERD symptoms and asthmatic symptoms from GERD.

Takeaway points

There is a two-way relationship between GERD and asthma, where they worsen each other. A majority of patients with asthma also experience GERD symptoms. Controlling and managing GERD symptoms can help control asthmatic symptoms.

If you are experiencing acid reflux and asthma symptoms, contact a gastro doctor near you to treat your digestive symptoms. If you are looking for top gastroenterologists or the best gastroenterologist in Bangalore, visit Bangalore Gastro Centre Hospitals in HSR Layout to get the best gastro treatment. The hospital is also the best gastro centre hospital to get an oesophageal manometry test in Bangalore to determine the underlying reasons for your acid reflux symptoms.
